PURA Wellness Studio

Client Consent, Release of Liability, and Acknowledgment of Policies.

1. Consent to Services

Client hereby voluntarily consents to receive body contouring and wellness services at PURA Wellness Studio, including but not limited to: massage therapy (manual lymphatic drainage, post-operative care, body sculpting), electrical muscle stimulation (EMS) treatments, ultrasonic cavitation, radio frequency (RF) skin tightening, fractional RF therapy, and any other non-invasive treatment modalities now or hereafter offered by Practitioner.


Client understands that the treatments are elective, cosmetic, non-surgical, and non-invasive. Client acknowledges that results are not guaranteed and that multiple sessions may be necessary to achieve optimal outcomes. Client further acknowledges that treatment outcomes may vary based on individual body composition, health status, and compliance with recommended aftercare.

2. No Guarantee of Results

Client acknowledges and agrees that Practitioner has made no representations or guarantees regarding the effectiveness or outcome of any services provided. Client understands that individual results vary and that there is no assurance of specific or measurable improvements.


3. Health Disclosure

Client certifies that they have truthfully and accurately disclosed all known medical conditions, previous surgeries, allergies, medications, implants (such as pacemakers), and other relevant health information that may impact treatment. Client agrees to update Practitioner of any changes to their medical history prior to receiving additional treatments.

Client understands that certain conditions, including but not limited to pregnancy, breastfeeding, active cancer treatment, infections, unmanaged chronic illnesses, or presence of medical implants, may contraindicate or restrict available treatments.

4. Assumption of Risk and Release of Liability

Client hereby voluntarily assumes all risks associated with the services provided, including but not limited to potential discomfort, skin irritation, temporary swelling, redness, bruising, or adverse reactions. Client agrees to release, indemnify, and hold harmless PURA Wellness Studio, its owners, practitioners, employees, agents, affiliates, successors, and assigns from any and all claims, demands, damages, actions, or causes of action arising out of or connected with services rendered or injuries sustained, whether known or unknown.


5. Photos, Videos, and Marketing Consent

Client consents to the taking of photographs and/or videos before, during, and after treatments for the purpose of tracking progress, client records, or marketing and promotional purposes (including but not limited to website, social media, advertisements).

Client understands that identifying information will be kept confidential unless express written consent is provided.

Client may opt out of marketing use of images by submitting a request in writing to Practitioner.

6. Payment Terms, Refunds, and Cancellations

Client understands and agrees to the following payment and cancellation terms:

(a) All sales are final. No refunds shall be issued for services rendered, packages purchased, deposits paid, or missed appointments.

(b) A valid credit or debit card must be provided and stored securely at the time of booking.

(c) A $50 cancellation fee will be charged to the card on file for cancellations made less than 24 hours prior to the scheduled appointment time, as well as for no-shows.

(d) Packages must be utilized within the timeframe specified at the time of purchase; unused sessions will expire and are non-refundable and non-transferable.


7. Chargebacks and Disputes

Client agrees not to initiate chargebacks or credit card disputes for services rendered, missed appointments, late cancellation fees, or prepaid packages. Any grievances must be communicated in writing to PURA Wellness Studio for review and potential resolution. Client acknowledges that initiating a chargeback without following this procedure constitutes a breach of this Agreement.

8. Entire Agreement

This document constitutes the entire agreement between the parties. No oral statements or prior written material not specifically incorporated herein shall be of any force and effect. If any provision of this Agreement is held to be invalid or unenforceable, the remainder of the Agreement shall remain in full force and effect.


By clicking the check box and signing below, Client acknowledges that they have read, understood, and voluntarily agreed to all of the terms, conditions, and policies set forth in this Agreement.
