Client Consent, Liability Waiver & Policy Acknowledgment

Consent to Treatment

I voluntarily consent to receive services provided by PURA Wellness Studio, including but not limited to manual lymphatic drainage, post-operative care, massage therapy, body sculpting, wood therapy, EMS, ultrasonic cavitation, radiofrequency (RF), fractional RF, and other non-invasive wellness or body contouring treatments offered by PURA Wellness Studio.

I understand that these treatments are elective, non-surgical, and cosmetic in nature.

Health Disclosure

I certify that the information I have provided is accurate and complete. I have disclosed all relevant medical conditions, allergies, medications, recent surgeries, pregnancy status, implants, pacemakers, and any other health information that may affect treatment.

I understand that failure to disclose relevant medical information may result in treatment being modified, postponed, or refused and may result in forfeiture of my booking retainer in accordance with PURA Wellness Studio policies.

I agree to notify PURA Wellness Studio of any changes to my health status before future appointments.

Treatment Risks & Results

I understand that all treatments involve varying degrees of individual response and that results are not guaranteed.

I understand that temporary side effects may include, but are not limited to:

• Redness

• Tenderness

• Swelling

• Bruising

• Skin sensitivity

• Increased urination

• Fatigue

• Temporary soreness

• Flu-like symptoms

• Temporary discomfort

I understand that treatment outcomes vary based on individual health, healing response, lifestyle, treatment consistency, and compliance with aftercare instructions.

Treatment Refusal

PURA Wellness Studio reserves the right to modify, postpone, discontinue, or refuse treatment if it is determined that treatment is unsafe, contraindicated, or not clinically appropriate.

If treatment cannot be performed due to undisclosed contraindications, inaccurate health information, or failure to follow required instructions, applicable booking fees, package sessions, or retainers may be forfeited according to studio policies.

Assumption of Risk & Release of Liability

I voluntarily assume all risks associated with the services provided.

To the fullest extent permitted by law, I release and hold harmless PURA Wellness Studio, its owner, employees, contractors, and affiliates from claims arising from known risks associated with treatment, undisclosed medical conditions, inaccurate health information, failure to follow aftercare instructions, or complications beyond the reasonable control of PURA Wellness Studio.

Photography & Media Consent

Photographs may be taken before, during, or after treatment for treatment documentation and progress tracking.

Marketing use of photographs or videos is optional and Declining marketing consent will not affect eligibility for treatment.

Booking Policy Acknowledgment

I acknowledge that I have read and agree to PURA Wellness Studio’s Booking & Treatment Policy.

I understand that:

• Booking retainers are non-refundable

• 48 hours means 2 full days before my scheduled appointment time

• Rescheduling with at least 48 hours notice transfers my retainer to a new appointment

• Cancelling an appointment results in forfeiture of the retainer

• Cancellations, reschedules, and no-shows within 48 hours result in forfeiture of the retainer

• Repeated cancellations, rescheduling, or missed appointments may result in restrictions on future booking privileges

I understand that it is my responsibility to review and understand all policies before booking.

Package sessions canceled with less than 48 hours notice, missed appointments, and no-shows are forfeited and deducted from the package balance.

Agreement

8. Entire Agreement

This document constitutes the entire agreement between the parties. No oral statements or prior written material not specifically incorporated herein shall be of any force and effect. If any provision of this Agreement is held to be invalid or unenforceable, the remainder of the Agreement shall remain in full force and effect.

By clicking the check box and signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Consent, Liability Waiver, and Policy Acknowledgment. I have had the opportunity to ask questions and voluntarily consent to receive treatment from PURA Wellness Studio.
